Complete the application and return it to the address listed below. Also submit product samples, catalogs, advertisements, price sheets and other company documentation and any plans for Huffy Licensed Products.

Note that this is an application only and does not imply an agreement between your company Huffy. A license agreement is not considered valid and in force until all the terms have been agreed upon and it’s signed by the appropriate representatives of all parties. The use of Huffy trademarks without a valid agreement is strictly prohibited.
