Student Participation Waiver, Disclosure Form and Media Release

ENGLISH:

I understand that all active sports may result in injuries. I release from responsibility the employees, officials, coaches, directors, members, volunteers and representatives of The Baseball Inc. (“The BASE”), Yawkey Boys & Girls Club of Roxbury of any liability, claims, or demands which may arise out of my child’s participation in practices, games, or any other functions or programs of The BASE and Yawkey Boys & Girls Club of Roxbury at The BASE Club House and Yawkey Boys & Girls Club of Roxburt gym and field/or any other fields and/or venues used by The BASE to deliver its programs and services.

In the event of a medical emergency, if a parent or guardian is not present, I give The BASE permission to administer first aid and/or call an ambulance for treatment and transport to a local hospital.

I also give permission for The BASE, Yawkey Boys & Girls Club of Roxbury, Yawkey Foundation and partners of The BASE, and the news media to photograph, videotape, and/or interview me and/or my child in conjunction with The BASE. I understand and acknowledge that any such photograph, videotape, and/or interview or portions thereof will be used for public view or distribution, and I grant The BASE and the news media any and all rights to use thereof without consideration.

I acknowledge that I have read and understand the above statements and that I am of legal age to execute this release and waiver. 

ESPAÑOL:

Entiendo que todos los deportes activos pueden resultar en lesiones. Libero de la responsabilidad a los empleados, funcionarios, entrenadores, directores, miembros, voluntarios y representantes de The Baseball Inc. (“La BASE”) de cualquier responsabilidad, reclamo o demanda que pueda surgir de la participación de mi hijo en prácticas, juegos o cualquier otra función o programa de The BASE, en The BASE Club House y/o cualquier otro campo y/o lugar utilizado por The BASE para entregar sus programas y servicios. 

En caso de una emergencia médica, si un padre o tutor no está presente, doy permiso a la BASE para administrar primeros auxilios y/o llamar a una ambulancia para recibir tratamiento y transportarlo a un hospital local. 

También doy permiso para que The BASE, los socios de The BASE y los medios de comunicación fotografíen, graben video y/o me entrevisten a mí y/o a mi hijo junto con The BASE. Entiendo y reconozco que cualquier fotografía, cinta de video y/o entrevista o partes de la misma se utilizarán para la vista o distribución pública, y concedo a The BASE y a los medios de comunicación todos y cada uno de los derechos de uso de los mismos sin consideración.
