Effective for the 2025 funding cycle, all Community Investment Fund grants must be for new projects or programs. UWC SDG will not fund programs it has funded previously. Community Investment Fund grants are for new projects and are for 2 years to help organizations develop the capacity to run and sustain a new program. This funding is not available to organizations for existing or historically funded UWC SDG programs.

Theory of Change & Project Outcomes
A Theory of Change links program outcomes and activities to explain HOW and WHY a desired change is expected to come about. It identifies what you are trying to achieve, what changes in people you expect to see, and how these changes will occur.

Please identify the group(s) that the selected project/program will support.
Note: we want to know your primary target group(s), not a comprehensive list of everyone you might serve with this program. Only check those most relevant to your project. We are moving towards emphasizing measurable outcomes and your project will be evaluated in terms of service to each population you identify below.



Reporting
A mid-year and final report will be required. The mid-term report will collect information on budget spent to-date, progress made towards achieving stated outcomes and number of participants in your program/impacted by your program. The final report should demonstrate your organization's analysis and interpretation of data collected including noting any unusual findings and future plans to build on/address the findings.

The information collected in this application will only be used to determine Community Investment Fun allocations for 2024-2025 through the United Way/Centraide Stormont, Dundas and Glengarry.
