Required Service Form

PLEASE NOTE

You only need to complete this application if we need to verify hours for you so you can prove your work to someone.

Common instances of verification include: 
  • People with mandated community service
  • Students required to perform service before graduation
  • Members of a religious organization who need service hours for a class
  • Anyone who needs a paper signed to verify hours served
If you are completing Court Required community Service Hours, you will need to upload your charge documentation and a photo of your ID in order to submit your application. Failure to disclose accurate and complete information about court charges may result in dismissal from your volunteer assignment and/or hours not being tracked or documented towards your hours requirement.

Required Hours Community Service Guidelines

As a Community Service Volunteer, you will be expected to follow all guidelines and expectations outlined in all written communications and confirmations from Crisis Assistance Ministry. In addition, we ask the following:

1.    For long shifts bring snacks, for colder items please bring your own cooler.  You will receive two 15 minute breaks or one 30 minute break if you work over 4 hours.
2.       Please remain productive during the entire time of your volunteer event, if you have completed a task please seek out staff to assign you another task.  If you are asked more than once to stay on task, you will be asked to leave and given a written warning.
3.       Sitting is not permitted unless you are on a break.  Stools and chairs are for elderly, special needs individuals, and volunteers that are not able to stand for long periods of time.  If you are not able to stand longer than 2 hours, please adjust your schedule to reflect your capabilities.
4.       After the third written warning your volunteer opportunities will be terminated. 
5.       Do not leave the assigned work area without first checking with a supervisor. A supervisor must be aware of the location for each volunteer.  Please let staff know when you are taking your break, if you do not we will assume you have completed your hours for the day.
6.       For safety purposes, please use all equipment as designed (ie; rolling racks, bins, pallet jacks).
7.       Respectful and appropriate behavior is always expected of everyone. Crisis Assistance Ministry reserves the right to define what behavior will not be tolerated.
8.       You may not receive visitors while performing Community Service hours.
9.       Shopping is permitted in the Free Store for residents of Mecklenburg County; however, you cannot shop on the same day as your Community Service.
10.    This is a smoke free building; smoking is not allowed on the premises (ie; parking lot, sidewalks).
11.    Please respect the privacy of others at our workplace. Please do not ask personal questions or give personal information to others at this work site.
12.    After three NO CALL, NO SHOWS (you have not removed yourself from the scheduled shift online) your time for community service will be terminated.  At that time, you will receive a letter of termination.  
13.    Cell phones are not permitted while working.  If you need to take a call or have an emergency let staff know, you can step outside or into the volunteer break room.
14.    You are able to listen to music with one earbud or headphone in one ear, both ears are not to be covered so you can hear staff or if an emergency arises.

Final Confirmation of Application
Once you submit your application, you will be asked to review the information that you have entered and add  your signature. You will also need to confirm your email address in a separate step for your application to be considered complete. After submitting this form, you will receive a confirmation email; in order to finalize your application, you will need to open the email, click the "Verify your Identity" link to validate your email address, and then your application will be submitted to Crisis Assistance Ministry staff for review.
