This page is a list of credit eligibility requirements. If these items are found on your credit report, you are not currently eligible, but you are still encouraged to submit the Pre-Application. Mountainlands Community Housing Trust (MCHT) can assist you in correcting these credit issues so that you may become eligible for an USDA Section 502 Loan. Of note, an applicant with an outstanding judgment obtained by the United States in a federal court, other than the United States Tax Court, is not eligible for a Rural Development loan or grant.

pdf format, ready for easy uploading. If your documents are too large, you will encounter an error message when you try to submit the application. It is up to you to provide the required documents pertaining to your situation.
- Copy of Driver's License or Utah State ID card (front and back) from all adult applicants who will be part of the household.
- Social Security cards (front) from all adult applicants who will be part of the household.
- Permanent Resident Alien cards (front), if applicable.
- Paystubs from all household members earning income. Include all income sources in the past 2 months. This means if you get paid once a month you would provide 2 check stubs, if you're paid every two weeks you would provide 4 check stubs. Each paystub must include employee and employer information, pay period dates, itemized gross earnings, deductions, and a summary including the year to date information.
- Two years of Federal Income tax returns (all pages) and W-2s for each wage earner.
- Official documentation supporting any other income (e.g., divorce settlement papers, child support, food stamps, unemployment statements, Social Security benefits, deferred school loans documentation, etc.).
- 2 months of statements from all financial institutions (e.g., bank statements, investment account, 401K, etc.).
- Signed Authorization to Release Information form.
