We take our obligations regarding your personal data under The Act seriously and as such the information which you provide in this form and any other information obtained or provided during the course of your volunteering ("the information") will be used solely for the purpose of compliance with legal and regulatory obligations and to identify and prevent potential risks.

CONSENT
If I participate in the volunteer programme:
- I consent to undertaking a DBS (if applicable)
- I declare that the information given in this application is honest and correct to the best of my knowledge
- There is no reason I cannot work with children (under 18’s) or vulnerable adults (if applicable)
- I understand that any false, incomplete or misleading information may lead to removal from the programme.
