All participants must complete the 2025 online application/waivers and submit a hard copy of the physician’s statement. Your physician must complete, sign, date the form, then you may return to LTR, Inc. via email, or by postal mail.

Spring semester students do not need to resubmit the application but must have a current physician's statement on file. Physician Statements are valid for one year.

The LTR Program Coordinator will contact you with a final schedule once space is confirmed. Invoices for program service fees will be sent by email.
