SPH Undergraduate Course Petition

This form is intended for undergraduate students in the Milken Institute School of Public Health (GWSPH) to submit petitions to their Program Director for one of the following exceptions:
  1. External Transfer Credit Evaluation: To request a review of previously completed coursework from another institution for potential approval toward fulfilling a core requirement or guided elective within the student’s intended or declared major/minor.
  2. Guided Elective Review: To request that a GW course not currently listed as an approved guided elective be reviewed for consideration, if the student believes the course aligns with and supports the learning objectives of their GWSPH major/minor.
NOTE:
  • This form is intended only for the exception types outlined above. It is not for requesting general transfer credit approval. Students seeking standard transfer credit evaluations should instead complete the Undergraduate Transfer Credit Approval Form. See the Blackboard Resource Page for more information.
  • This form should not be used by current GW students to request permission to complete core requirements at another institution. Please refer to your program guide for more information.
  • This form should not be used to request a waiver of required courses or prerequisite requirements. Please refer to your program guide for more information.
  • All submissions will be reviewed by the Program Director for the student’s undergraduate program. Students will be notified of the outcome via email by an academic advisor.
If you are unsure whether this form is appropriate for your situation, please consult your academic advisor before proceeding.
