Welcome to Stripey Stork's Santa Stork request form - Please use this form to request Christmas gifts for families. The Santa Stork project 2025 is open to families in hardship living in Surrey, or the London Boroughs of Sutton or Croydon. Requests are accepted subject to availability.

Please note we are no longer accepting spreadsheets for Santa Stork requests. 

Please complete one form for each family, indicating the gift requested for each child. Please note that only one Christmas gift can be requested per child to help us ensure fair and consistent support for every family that’s referred to us.

The data is collected via our referral partners, and is only used in aggregated form to assess the impact of our work. The system we are using to manage referrals data is Salesforce, the world’s #1 cloud-based CRM system. It has robust data security measures in place. If there are no gifts of the type selected we will provide a suitable alternative based on age and gender. At the end of this form there is a comments box which you to add any notes about likes and dislikes to guide us.

Delivery Options:

We have existing arrangements with some referrers - if you know your organisation or school regularly receives deliveries from us, you should be able to find your delivery location on the 'internal only' list. Contact us if you're not sure.

Delivery slots are limited and we cannot guarantee availability.
