Welcome to the Registration Page for the Arts for Learning at Goodnow Afterschool Program.  The program is being operated by Arts for Learning Maryland (formerly known as Young Audiences of Maryland) and is offered to students CURRENTLY in grades Kindergarten - 8th.


NOTE: Applications are reviewed on a rolling basis, and registration is first come, first served. The program fills up quickly, so apply today!

If you can't locate your student's ID Number, it is highly recommended that you call their home school to request it, and then complete this registration form. Registration/enrollment is on a first come, first served basis and there are a limited number of slots.

Dear Arts for Learning at Goodnow Community Center Families, 


Welcome to Arts for Learning at Goodnow Community Center! We are honored and excited to work with your children! Please review and plan to adhere to the following policies and procedures:

 With a mission and vision centered in community, compassion, cultural understanding, academic excellence, and arts and recreational enrichment, we look forward to bringing together community members of all ages in meaningful and joyful experiences all year long at Goodnow, including youth summer and after school programs and community events.

AFTER-SCHOOL PROCEDURES:


  1. Attendance: 

  • Schedule: The program is held Monday through Friday from September 8th through May 29th from 2:00 PM to 6:00 PM.

  1. Timeliness: Please plan to arrive on time for drop off and pick up. Please call if you will be arriving later than 6 pm. There will be a grace period from 6:00 p.m. to 6:10 p.m. for your student's first and second late pickup. After that time, the following consequences will be enforced:


  • Grace period for the first late pick-up with advanced call

  • Documentation for the second late pick-up with advanced call.

  • Third late pick-up with the advanced call: program dismissal.

  • Documentation and a restorative meeting for the first late pick-up with no call.

  • Dismissal from the program after the second late pick-up with no call.


  1. Dismissal: Only the approved parent or guardian listed on your application will be permitted to pick up your child for safety reasons. If there have been any changes, please communicate them in writing to goodnow@artsforlearningmd.org. 

Etiquette: All students, parents, and guardians must treat all staff, students, and partners respectfully. This means no kicking, fighting, shoving, hitting, or spitting on someone. No cursing, verbal threats, or gestures of harm will be tolerated.  The following consequences will be implemented:
  • 1st  Incident: Verbal mediation and reflection time using restorative practices. 


  • 2nd Incident: Email sent to parent or guardian. A restorative family conference will be scheduled with the student, parents, Goodnow Site Director, or staff member.


  • 3rd Incident: The student will be dismissed from the After-School Program.


  1. Meals: Please pack your child a water bottle each day. A supplemental dinner and snack will be served daily and stored in our commercial refrigerators. 


  1. Attire: Students should come to the program in comfortable clothing. Sneakers or sandals without heels are preferred. We will engage in crafts, yoga, fitness, and field trips. We recommend a light jacket for cooler days and a change of clothes (including extra undergarments) in a clearly labeled bag. 


  1. Medicine: If your child needs medication administered, please bring it to the program in a labeled ziplock bag with clear instructions. This medicine will be kept in a locked medical box and will be returned on the last day of the program. 


  1.  Outside Items:  There are plenty of activities for the Students to engage in on-site. Please leave personal toys, cell phones, and other electronics at home. 


  1.  Winter & Spring Performance Opportunities:  Our Students will work on winter and spring performances that will take place in December and May. As we get closer to the programs, students will be needed for rehearsals. Please support this effort with consistent attendance and no early pick-ups as much as possible.

  2. Family Engagement: There will also be family engagement events, workshops and performances  that take place throughout the year. In the spirit of community building, we invite you to spend quality time with the Goodnow students, staff and families learning, growing and having fun together.

Programming Questions



Consent and Release
I am the Parent/Guardian of
A minor child (the “Student”). Arts for Learning Maryland. (“Arts for Learning”) is the operator of the Goodnow After-school Program.

By checking the CONSENT box below and signing this document, I agree and consent to all of the following terms and conditions:

1. I give permission to the staff in the Goodnow Afterschool Program to administer basic first aid and/or CPR to my child and/or take the Student to a hospital to secure medical treatment when I cannot be reached, or when delay would be dangerous to my child's health.

2. I certify my understanding that I will receive policies and procedures and related information about the Goodnow Afterschool Program. After I enrolled my child in the Goodnow Afterschool Program, I agree to follow the policies and procedures described.

3. I give Arts for Learning Maryland, Inc. (“Arts for Learning”) permission to access and keep copies of my child's/children's application record(s), Arts for Learning will keep these data confidential and use them only for analysis and program planning. I understand that Arts for Learning may share data collected during the program, including attendance, demographics, and academic outcomes, with program funders and sponsoring organizations; however, any information shared will not specifically identify program participants. 4. I give Arts for Learning permission to contact me (using telephone, email, mail, SMS text message, or robocall via telephone) using the contact information I provided about the status of enrollment, my child's attendance, upcoming events and programs, schedule changes and program updates.

5. I certify my understanding that I will be given a Field Trip Permission Form to authorize my child's participation in any field trip or other off-site activity planned by the program and that alternate care will not be provided should I choose not to grant such permission for the Student’s participation. Additionally, I acknowledge that the Student’s participation in Field Trips is at the sole discretion of the Arts for Learning Team.

6. I release Arts for Learning as an organization, as well as its employees, contractors, volunteers, and agents (collectively, the “Arts for Learning Team”) from any liability suit or claim for property damage or loss, or personal injury to the Student, except to the extent such claims arise directly from the gross negligence of Baltimore City Public Schools, Arts for Learning and/or the Arts for Learning Team. 7. Arts for Learning takes great care in capturing thoughtful, high-quality images of students’ incredible artwork, their pride, excitement, determination, friendship, and joy. I understand that the images, video, and materials collected during the Goodnow Afterschool Program are vital for building awareness of the program in the community and securing funding so that it can continue to grow and remain available at no cost to students. I give permission for the Student to be included in the documentation/promotion of Arts for Learning and its partners, including photographs, audio/visual/video recordings, interviews, reproductions of academic and artistic work, participation in surveys, and written quotations or descriptions of activities. I also understand that resulting materials may be exhibited before the community, fundraising, or other groups and individuals and/or included in media articles, partner publications, and Goodnow Afterschool Program funder promotional materials.   

8. I certify that the Student will attend every day of the program except in the event of an emergency. I acknowledge that students who miss more than 3 days of the program may be removed so that their spot can be given to another student


9.  I certify that the entered information is true and correct to the best of my knowledge.


PLEASE READ THIS ACKNOWLEDGMENT OF RISK, WAIVER, AND GENERAL RELEASE OF CLAIMS (THIS “RELEASE”) CAREFULLY. BY SIGNING THIS RELEASE, YOU WILL GIVE UP CERTAIN LEGAL RIGHTS. IF YOU DO NOT UNDERSTAND ANYTHING IN THIS RELEASE, YOU SHOULD SEEK YOUR LEGAL COUNSEL'S ADVICE BEFORE SIGNING BELOW.

 

In consideration of providing your child/children with recreational, sports, and physical activities and being permitted to attend Arts For Learning Maryland’s Goodnow Community Center programs, the undersigned (“you”), on behalf of yourself and the other persons described below, hereby agree to the following:

 

Acknowledgment of Risk. You acknowledge there is an inherent risk in your child/children attending Arts For Learning Maryland’s Goodnow Community Center programs. You further acknowledge that such risk includes, but is not limited to physical injury, illness, death, or damage to personal property. You acknowledge the risks inherent in the time you are present in the program. You and your child/children assume all risk of any injury, damage, or loss. You acknowledge that Arts for Learning Maryland would not be willing to provide such activities to or permit your child/children to attend unless you enter into this Release and that your consent to provide such activities and/or permit your child/children to attend is good, valuable and legally sufficient consideration for this Release.

 

Waiver and Release Claims. You irrevocably and unconditionally waive, relinquish, discharge, release, and covenant not to sue Arts for Learning Maryland and each of their respective affiliates, owners, directors, managers, members, officers, employees, representatives, board members, agents, assigns and insurers (collectively, the “Waived Parties”), from any and all rights, claims, demands, causes of action, damages, liabilities or losses, whether known or unknown, that your child/children have or incurs, or may have or incur, that arise during, result from or are related to your child/children’s participation in its programs  WHETHER CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OR OTHER FAULT OF ANY OF THE WAIVED PARTIES. THIS IS AN ABSOLUTE WAIVER AND RELEASE.

 

Disclaimer of Liability. In no event shall any Waived Party be liable for any direct, indirect, incidental, special, exemplary, punitive, or consequential damages, however, caused and on any theory of liability, whether in contract, strict liability of tort) arising in any way out of your child/children’s participation in the Arts For Learning Maryland programs even if such Waived Party has been advised of the possibility of such damage. This disclaimer of liability applies to any damages, injuries, or losses including, without limitation, personal injury, death, or property damage, under any cause of action



Additional Representations. You acknowledge you have had an opportunity to object to any or all the terms described herein and, after careful consideration, fully understand the extent of the waiver represented by such terms in their aggregate and waive your and your child/children’s right to bargain for different terms. You understand that if you later learn that any fact you believed to be true at the time you signed this Release is later found to be incorrect, you and your child/children nevertheless are bound by this Release. 


YOU HAVE READ THIS RELEASE THOROUGHLY. YOU SIGN THIS RELEASE VOLUNTARILY ON BEHALF OF YOURSELF, YOUR MINOR CHILD/CHILDREN, YOUR AND THEIR HEIRS, NEXT OF KIN, ASSIGNS, PERSONAL REPRESENTATIVES, AND RELATED INDIVIDUALS. NO ONE HAS MADE ANY REPRESENTATIONS, STATEMENTS, OR INDUCEMENTS THAT CHANGE OR MODIFY ANYTHING WRITTEN IN THIS RELEASE.


I understand and agree to comply with all the following requirements for my child’s participation in the In-Person Arts for Learning at Goodnow Arts Enrichment Program.

  • My child will follow the Code of Student Conduct, as well as all rules for wearing a mask and social distancing, set forth in the City Schools Health and Safety Guide, which is grounded in science-based, expert guidance and real-time experience from in-person operations in City Schools and across the United States and in other countries (https://www.baltimorecityschools.org/safety-procedures)
  • I will not send my child to the Goodnow After-School Program if they have symptoms of a COVID-19 like illness, have been diagnosed with COVID-19, are waiting for test results, or have been in close contact (within 6 feet for at least 15 minutes total over a 24 hour period) with anyone diagnosed with COVID-19 or suspected of having COVID-19 in the past 14 days. I also agree to report to City Schools that my child has a confirmed or suspected case of COVID-19. 
COVID-19 Guidelines and Expectations Signs and Symptoms of COVID-19:
New onset cough, shortness of breath OR At least 2 of the following:  
    • Fever of 100.4 or higher 
    • chills 
    • shivering 
    • muscle pain 
    • sore throat 
    • headache 
    • loss of sense of taste or smell
    • Gastrointestinal symptoms (nausea, vomiting or diarrhea) 
  • I will review symptoms with my child and monitor my child’s symptoms every day that my child attends the Goodnow After-School Program
  • If my child becomes ill during the Goodnow After-School Program, I will ensure they are picked up from school promptly and will follow-up with an authorized health care provider/health department and comply with recommended quarantine or isolation as directed. If my child is ill, I understand that a release to return to in-person activity from an authorized health care provider will be required.
  • I will ensure that a parent or guardian remains with my child until they complete their health screening. If my child does not pass the health screening, a parent or guardian will be there to take my child home.  
Students must be free of fever without the use of fever reducing medications for the period of time directed by the Maryland Department of Health’s current guidelines. Please consult your health care provider or the Maryland Health Department (Dial 211) with specific questions about COVID-19.

If City Schools is able to provide rapid COVID-19 testing to participants in the Goodnow Summer Program, I consent to my child’s testing onsite by health professionals working in collaboration with City Schools and that tests results will be shared with the parent/guardian, City Schools, and the Baltimore City Health Department. 

I understand that failure to comply with the above rules may require City Schools to take steps necessary to protect the health and safety of my child, as well as other students and staff.

I have reviewed information regarding COVID-19, including but not limited to these documents:

Based on my review of this information, I am aware that there is a risk of being exposed to COVID-19 by participating in the Goodnow After-School Program. I am also aware that exposure can occur either directly or indirectly, whether or not a mask is worn, and notwithstanding reasonable efforts by Arts for Learning at Goodnow, the host site and/or Baltimore City Public Schools to mitigate exposure. 


I have considered my child’s and family’s personal health risk in the decision to attend the Goodnow After-School Program. I have independently evaluated and reviewed the risk of being exposed to or infected by COVID-19, and I have determined to allow my child to participate in the Goodnow After-School Program with full knowledge, acceptance, and assumption of all the above risks.

In the event that programming has to be transitioned to virtual as a result of any COVID-19 related scenario, I will:

1. Contact my child’s teacher with any questions or concerns; 
2. Support my child by:
  • Establishing a physical space where my child can plug in and engage in virtual programming; and
  • Encouraging the completion of activities and assignments.
3. Ensure my child is engaging in virtual programming in a manner that reflects community values including:
  • Using the chat feature of Zoom for conversations only related to the lesson, and as instructed by teachers;
  • Refraining from posting any identifying information including their full names, phone numbers, email or home addresses, as well as, avoid posting or sharing of any website or link in the Zoom chat feature; and 
  • Refraining from recording or capturing any image during delivery of programming.

By signing below, I represent and certify that I am the parent or guardian of the Student, at least eighteen (18) years of age, and fully competent and authorized to sign this Consent and Release.
