FY26 Materials Request Form

Hello! Roundabout Teaching Artists should use this form to request materials for workshops and residencies. Please allow up to two weeks from form submission to the item arriving at your indicated site.

Before you hit submit, please double check that:

You received approval from the appropriate program manager to order your requested materials
If applicable, the total sum of materials falls within your allocated budget
Your materials are expendable (i.e. screws, paint, fabric, paper) and not equipment (i.e. power tools)
