NEW joint initiative between Encircle Families and Diverse Ability Incorporated launching in 2026!
This collaborative pilot project aims to:
Provide comprehensive Family Navigation Support for families, and students with disabilities between the ages of 14-22 years old who are navigating transition to adulthood
- Assist families to set and achieve goals related to transitioning into adulthood
- Provide transition expertise in group and/or individual settings on the interplay of key systems and transition resources
- Utilize the family voice, expertise and input throughout the project
- Build capacity in families and students transition knowledge
