By completing this form, you provide consent for Centrepoint to undertake due diligence checks on your organisation and to report on how you have contributed the defined outcomes of the Grant Programme (see programme guidance document for applicants).

By completing this form you provide consent for Centrepoint to share your organisation’s details with the funder and for Centrepoint in its general promotion of the Grant Programme.

Completing this form does not constitute a grant offer. Centrepoint will write to confirm if a grant offer is being made following satisfactory completion of the grant application and due diligence process.

You are advised to first read the grant application guidance before completing this form. Organisations should only submit one application for all of their services.

This form should be submitted by 5pm, 30th April 2026.

This grant is for the provision of items for a young person aged between 16 and 25 years old who is receiving support from a Centrepoint Partnering Scheme member organisation to move on to their own accommodation. You will be applying for £400 per young person.

Please ensure you have read the guidance material to understand what criteria the items being purchased by the young people will need to meet to be considered eligible items of expenditure.

• Your two most recent sets of annual accounts. These must be audited or signed by a financial officer or treasurer
• Your governing documents: constitution/memorandum and articles of association

Please ensure the required documents are uploaded or submitted to Centrepoint, as we will not be able to assess your application without them.
