MPACT often takes photos, verbal statements, or videos for use in a variety of ways. They may be used on our website, on social media, and at fundraisers and they serve as wonderful positive publicity for our organization. To respect each individual and his/her family, we ask that permission be granted for the use of yourself/your child in these media and verbal publications. Neither you nor your child will be identified by name on the MPACT website or in any publication which we might use the photographs. Please sign a photo release for yourself and/or your child who is under age 18.
By attendance at the MPACT sponsored program, the Attendee (or hereinafter Attendee's parent, on Attendee's behalf if Attendee is under 18 years of age) expressly agrees that MPACT (its agents, employees, and volunteers) shall not be liable for any damages arising from personal injuries sustained by the Attendee as a result of any injuries sustained while coming to attend, while attending or while leaving the attendance of the MPACT sponsored event (collective hereinafter "attendance").
Attendee also assumes all risks associated with any food products provided, caused in preparation, delivery or consumption. Whether or not the Attendee's damages stem from MPACT's own fault or negligence, Attendee does hereby fully and forever release and discharge MPACT and all employees and agents from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out of the attendee's attendance at the MPACT sponsored event.
