I acknowledge that any personal information supplied to SARA is done so voluntarily to facilitate the processing of this application. I furthermore acknowledge that the information provided by me, is to the best of my knowledge both true and correct, and I understand that any incorrect or inaccurate information or documentation submitted may adversely affect the manner in which SARA processes my application to be a mentee.

The information you are voluntarily providing will be used to help assist us in achieving our mentorship programme goals for SARA.

The information collected will be used in accordance with the POPI Act and will only be shared with the SARA Mentorship Committee.

Terms and Conditions:

I hereby confirm the following:

1. I am responsible for ensuring that my membership fees are settled for the year by 31 January.

2. I consent to SARA collecting and using my application data and feedback for the purposes of matching, monthly check in administration, and programme evaluation.

3. I have permission from my line manager to participate in the programme.

4. My mentor is assigned based on the information provided in my application, as well as the availability of mentors, reassignment is not guaranteed.

5. I will fully commit to the programme within the rules and guidelines set out by SARA, with the understanding that I am responsible for setting up the meetings with my mentor, as well as maintaining respectful interaction with him / her, with due regard for his/her time and commitment to the programme on a voluntary basis.

6. I understand that the programme is intended to enhance my personal and professional growth and is not intended to be a training / consultation session.

7. Should I experience any difficulties during the programme, e.g. be unable to continue with the programme for any reason (personal or professional), experience compatibility challenges or any aspect which may hamper the successful completion of the programme, I will immediately inform the SARA relationship manager.

8. Should I fail to meet the requirements as set out above, my employer may be informed that I did not complete the programme due to negligence / lack of commitment on my part.
