Congratulations on receiving an Artistic Assistance Grant from Alternate ROOTS. Per your grant agreement, a final payment will only be issued upon completion of this final report form. You should only complete this form if you have spent all of the funds you've already received and have either completed or made significant progress towards the completion of your project.
