If you are applying for a position in the kitchen, as office/store staff, as health officer, or as utility crew, you will be interviewed by phone. An executive staff person will contact you; do not set up an interview on the dates below.

I authorize Cazadero Performing Arts Camp to investigate all information contained in this application. I authorize the prior employers to give you any and all information concerning my previous employment and any pertinent information they may have, personal or otherwise, and release all parties from any and all liability for any damage or injury that may result from furnishing the same to Cazadero Performing Arts Camp. 

If Cazadero Performing Arts Camp employs me, I agree to conform to the rules and regulations of Cazadero Performing Arts Camp. I understand and agree that, except for employment at-will status, my hours, working conditions, job assignment(s), and compensation rate(s) are subject to change by Cazadero Performing Arts Camp. I understand my employment can be terminated, with or without cause and with or without notice at any time at the option of Cazadero Performing Arts Camp or myself. Cazadero is a drug-free and alcohol-free camp. I understand that contact with minors under any form of intoxication will be grounds for immediate dismissal. 

I understand that photographs may be taken during camp and staff activities. I authorize Cazadero Performing Arts Camp to use photographs of myself/my child for promotional materials.
