I understand that if I am employed, falsification of this application including falsification by omission or supplying misleading information may result in discharge regardless of when such falsification is discovered. I authorize Cazadero Performing Arts Camp to investigate all information contained in this application. I authorize Cazadero to reach out to my prior employers to confirm employment information allowable under federal and state law, and release all parties from any and all liability for any damage or injury that may result from furnishing the same to Cazadero Performing Arts Camp.

If Cazadero Performing Arts Camp employs me, I agree to conform to the rules and regulations of Cazadero Performing Arts Camp. Cazadero Performing Arts Camp is a smoking-, vaping-, drug- and alcohol-free work environment. I understand that contact with minors under any form of intoxication will be grounds for immediate dismissal.  I understand that employees must complete an I-9 and provide proof of authorization to work in the United States. Employees must also submit fingerprints for a Live Scan criminal background check.  I understand that Cazadero Music Camp requires all employees to represent that they have not been convicted of any sex crime, drug crime, or serious or violent felony.
