1. Please have the supervisors of the Cornell team members to use this form to express their endorsement of the proposal.
2. A statement of collaboration, or partnership agreement, must be signed by the community partner team member. While the format of statements of collaboration can be varied, they should describe the role that the community partner expects to play in the collaboration, benefits their organization or community will receive and how they will interact with Cornell students. Common formats include letters from community partners on their organization's letterhead and bulleted Memos of Understanding.

Please outline how your project will meet the Einhorn Center’s four criteria of community-engaged learning:
1. address a specific community interest, problem or public concern, ideally identified by a community partner;
2. include Cornell undergraduates working with and learning from a community partner;
3. provide Cornell undergraduates with educational content that connects to and integrates with their community engagement; and
4. facilitate critical reflection for Cornell undergraduates.
