Breathe Hope Nebulizer Request and Waiver

Thank you for choosing Global Links to request nebulizers for your clients!

  1. Global Links is a non-profit organization. We receive new and gently used nebulizers from consumers, which we thoroughly check and clean then package with all new accessories.

  2. Free and charitable clinics or other organizations can request nebulizers on behalf of their clients. Global Links cannot ship nebulizers directly to patients.

  3. Nebulizer requests must be made in quantities of 1, 3, and 5’s for a minimum order of 1 unit and a maximum of 30 nebulizers per application/request.

  4. Nebulizers from Global Links include 1 replacement filter, 1 nebulizer kit containing tubing and mouthpieces, and a pictogram instruction sheet.

  5. Pediatric masks can be requested as needed.

  6. Global Links processes nebulizer requests in the order in which they are received. Agencies will receive a notification that their request was received and instructions on paying any fees. Payments can be made via check or online with a credit card. Payment must be received before nebulizers can be shipped.

  • Provide basic information about you and your organization
  • Indicate how many nebulizers your organization would like
  • Read and agree to a waiver of liability

Each unit includes one replacement filter, 1 nebulizer kit containing tubing and mouthpieces, and a pictogram instruction sheet.

Nebulizers are only available to organizations with licensed prescriptive or dispensing authority.


For no additional charge, you may include pediatric masks if needed. If you would like to receive pediatric masks please submit how many below. Note that there is a limit to one pediatric mask per nebulizer.

Waiver to Receive Materials from Global Links

Global Links donates surplus supplies, furnishings and/or equipment, for use in programs of assistance to underserved populations. Said materials are donated in "as is" condition, without any warranties or representations as to their condition, functions, performance, or safety. The recipient hereby takes full responsibility for their use and waives and relinquishes any present or future claim which it may have against Global Links related in any way to the use of these materials, including but not limited to economic claims based on performance or non-performance of said supplies, claims for personal injuries related to the handling or use of said supplies, and claims related to the subsequent distribution overseas of said supplies.

The recipient further agrees to indemnify GLOBAL LINKS and hold it harmless from any and all third party claims and any damages, judgments, settlements, or costs related to, or arising from its donation of said supplies including but not limited to reasonable attorneys' fees.
