I, the parent or legal guardian of this camp participant in the 2019 State Championship Football Camp, do hereby grant permission for his/her participation in any and all camp activities. I agree to assume all risks (including food allergies) and hazards incidental to participation in a football camp.  I do hereby waive, release, absolve, indemnify, and agree to hold harmless, the 2019 State Championship Football Camp, Wesley Chapel Weddington Athletics Association, and the coaches, sponsors, volunteers, and individual participants for any claim arising out of an injury to my child, whether the result of negligence or any other cause.

I hereby grant the 2019 State Championship Football Camp and its sponsors, permission to use my likeness and my participant’s likeness in a photograph and/or video in any and all of its publications, including website entries, without payment or any other consideration.

I understand and agree that these materials will become the property of the 2019 State Championship Football Camp and its sponsors and will not be returned.

I hereby irrevocably authorize the 2019 State Championship Football Camp and its sponsors to edit, alter, copy, exhibit, publish or distribute this photo and/or video for purposes of publicizing the 2019 State Championship Football Camp or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph and/or video.

I hereby hold harmless and release and forever discharge the 2019 State Championship Football Camp and its sponsors from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I ACKNOWLEDGE THAT MY ELECTRONIC SIGNATURE CONFIRMS THAT I HAVE READ, UNDERSTOOD, ACCEPTED, AND AGREED TO THE ENTIRETY OF THIS DOCUMENT. MY ELECTRONIC SIGNATURE IS LEGALLY BINDING AND SERVES AS MY CONSENT FOR MY CHILD’S PARTICIPATION IN THE 2019 STATE CHAMPIONSHIP FOOTBALL CAMP ACTIVITIES.
