The D1 Transition Learning Collaborative (D1 TLC) is seeking parents and caregivers to share their experiences and ideas about supporting youth and young adults with disabilities as they prepare for adulthood.

During this online focus group, we want to learn what is working, what is missing, and what resources and supports families need related to school, college and career readiness, independent living, and transition to adulthood.

The focus group will be facilitated by Measurement Resources, without D1 TLC or Parent Center staff present, so families can speak openly and honestly.

Your feedback will help shape future D1 TLC resources, trainings, and supports for families across Region D1.
