Quick Look Award Eligibility:  Can you answer "Yes" to all of the following?  
  • The program, service or initiative that you are submitting has been in operation for at least one (1) or more years.  
  • The program, service or initiative has successful achievements worthy of consideration for this Award. You are not submitting a grant application asking for new or continuation funding.  
  • Your organization is a non-profit, public organization or Tribe in the US. (Gather your organization status verification now so that you upload it the end of the application.)
  • You have proof that your program was successful, e.g. client satisfaction survey or other evaluation data, first person testimonials, policies adopted/legislation passed or similar.
Application Guidance:
  • The option to “Save my progress and resume later” is available at the top of the form. If you choose to use this function, you need to create a login and password. Save your password! FCA cannot assist with lost passwords.
  • When completed, click “submit” on the bottom of the form. After clicking “submit,” you may review and make any final edits. Click “confirm” to finalize submission.
  • You will receive an email with a copy of your application as confirmation.
