Step 1 of 10
Welcome, let's make our time together count.
A few questions before we meet, so our conversation can be about you, not paperwork.
At Capital Investment Advisors, we believe a great financial plan is about more than money. It's about the people, experiences, and goals that matter most to you. This questionnaire helps us understand your full picture before we meet, so we can spend our time on what matters. If possible, we appreciate you completing this at least 2-3 days before our meeting, so we have time to review the information.
What to expect
The essentials come first and take about 10 to 15 minutes. Pages 9 and 10 are optional and add about 5 to 10 minutes each. Completing this questionnaire helps CIA prepare for your meeting with an advisor and does not, by itself, create an advisory relationship with Capital Investment Advisors.
A few things to know
There are no right or wrong answers. Skip anything you're unsure about. You can save your progress and come back anytime.
Helpful to have nearby
A recent investment or retirement statement, a rough sense of your account balances and debts, and, only if you'd like us to review them, a recent tax return or estate documents.

Step 2 of 10
First, the essentials.
Just enough to know who we're meeting and how to reach you.

MM/DD/YYYY

Are you planning together with a spouse or partner?
If yes, check the box below and we'll ask a few of the same questions about them. If it's just you, skip ahead.

MM/DD/YYYY

Step 3 of 10
A bit about your household.
This rounds out the family picture and helps us tailor what we cover.

Step 4 of 10
Let's map out your accounts.
Add each account you have; estimates are perfectly fine. Have a lot? Add as many as you need, or just upload a recent statement and we'll take it from there.

Your Accounts

By uploading documents here, you authorize Capital Investment Advisors to receive and store them for the purpose of preparing for your upcoming meeting. Documents are handled in accordance with our Privacy Notice. Your information is not sold or shared with unaffiliated third parties for marketing purposes; it may be processed by service providers who assist us in operating our systems, all of whom are contractually obligated to protect it. Please avoid including full Social Security numbers or account numbers in any uploaded file.

Step 5 of 10
What you owe.
A quick look at any debts completes the picture. Estimates are fine; leave blank what doesn't apply.

now this:

Step 6 of 10
Money coming in.
Roughly what comes in each year. A ballpark is all we need; leave blank anything that doesn't apply.

Step 7 of 10
Spending, today and in retirement.
This helps us see whether your plan is on track.

Step 8 of 10
Your team, and what you're hoping for.
Who else helps you, and what would make this worthwhile for you.

Step 9 of 10
What's on your mind?
At CIA, our goal is to help coordinate every aspect of your financial life, ensuring all of the pieces work together toward your goals. Check anything you'd like to talk about: it shapes our conversation.

Step 10 of 10
Beyond the numbers.
We plan for a life, not just a portfolio. These help us understand what a good life looks like to you. If you're a couple, answer together or from your own view; whatever feels right. As a reminder, answering these questions is helpful, but completely optional.

Why we ask: This helps us understand your overall wellbeing as part of the full picture of your life, not just your finances. Optional, and not used for medical purposes.
