* Please note that two important application requirements involve the input of others and may require more lead time to secure. Both of the following must be complete with signatures and uploaded at the bottom of this application at the time of submission.1. Please have the supervisors of the Cornell team members to use this form to express their endorsement of the proposal. Supervisors/chairs should include why the proposal is beneficial for their department/unit.
2. A statement of collaboration, or partnership agreement, must be signed by the community partner team member. While the format of statements of collaboration can be varied, they should describe the role that the community partner expects to play in the collaboration, the benefits their organization or community will receive and how they will interact with Cornell students. Common formats include letters from community partners on their organization's letterhead and bulleted Memos of Understanding.
