It is our standard practice to ask all those interested in volunteering with us whether there are any special access requirements we need to take into account during the recruitment process. If you need to receive or submit any recruitment information in alternative format (e.g., large print, or on disk/CD), please let us know.

The principle that the welfare of children and young people must be paramount consideration is at the core of child protection work in the UK. Beanstalk fully supports this principle. Therefore, everyone connected with Beanstalk who will come into contact with children must complete and sign this declaration.

The DBS filtering guidance is available at:

If you have answered 'Yes' or 'Don't know' to any of these questions you will be contacted by your local Beanstalk area office to discuss before your application can be progressed.

I declare that the information I have given on this form is complete and accurate and that I am not banned or disqualified from working with children nor subject to any sanctions or conditions imposed by The Disclosure and Barring Service or a regulatory body.

Please note

All volunteers will be required to complete our forms relating to equal opportunities and provide suitable contact details for us to provide two references.

If you are applying to volunteer in a role which is classed as regulated activity, according to guidance from the Disclosure and Barring Service, you will be required to undertake a DBS Check via route 1 or 2 during our recruitment process. Certain roles also require the completion of a disqualification by association form.

Data Protection Statement

The information which you give when completing this form and subsequent recruitment documents will be used in accordance with the Data Protection Act 1998 for the following purposes: to enable Beanstalk to create an electronic and paper record of your application; to enable the application to be processed; to enable the organisation to compile statistics, or to assist other organisations to do so, provided that no statistical information that would identify you as an individual will be published.
