CanIL Waivers

This form contains two separate waivers for you to read over and sign:

The first waiver is the Community Life Waiver. CanIL places a high value on community, and as a result, we host many events and activities to help build community life. The summer session has a particularly high concentration of community life events. The Community Life Waiver is a Release of Liability, Waiver of Claims, and an Assumptions of Risk and Indemnity Agreement.

The second form is the Photo Consent Form. As with most group events and activities (even online ones), photos are often taken. This consent form allows you (the participant) to have a say in how photographs (containing your image, taken at CanIL events, by someone representing CanIL) may or may not be used.

If you are married and your spouse is also participating in CanIL events, please have your spouse sign these waivers as well. Also, if you have adult children (over the age of 18) with you at CanIL, please have them sign individual forms.

You will only need to sign this form once, and you will receive a copy of this form with your signature at the email address you provide below. It is your responsibility to let us know if your responses change for any reason. If you need to change your response, you can fill out a new response, which will override your previous response(s).

The Community Life program during the summer session at CanIL, is an integral part of the growth and training that you will experience here. This involves camping trips, brunches, Snax and Pix, t-shirts, and outings of all sorts. Summer community life is additionally and significantly subsidized by CanIL because we see great benefit in being together outside the classroom as much as in the classroom.

All Summer@CanIL students, faculty and staff must sign the Community Life Waiver on behalf of themselves and any participating dependent children. Spouses must also sign the Community Life Waiver.

If you have questions related to the Community Life Waiver or this requirement, please talk with the Director of Community Life.

You must read this agreement, sign consent, and submit your response before participating in any CanIL events.

WAIVER OF CLAIMS FORM

Release of Liability, Waiver of Claims

Assumptions of Risk and Indemnity Agreement

By signing this document you will waive certain legal rights, including the right to sue.

PLEASE READ CAREFULLY!!

TO: Canada Institute of Linguistics and its Board of Governors, directors, officers, employees, agents, independent contractors, subcontractors, representatives, successors and assigns, and all volunteers, sponsors, officials, and other persons in any way involved or connected with CanIL (all of whom are hereinafter collectively referred to as "the Releasees").

ASSUMPTION OF RISKS

I am aware that participation (including that of dependent children if applicable) at CanIL and CanIL Activities (both on and off campus) involves risks including but not limited to:

• Any manner of injury, illness or death resulting from terrain conditions; falling or impacting surfaces;

• Other hazards (e.g. motor vehicles, theft, vandalism); negligence on the part of other participants, officials, volunteers, organizers, instructors, and negligence on the parts of releasees, including the failure on the part of the releasees to safeguard or protect me from the risks, dangers and hazards referred to above.

On behalf of myself (including dependent children if applicable), I freely accept and fully assume all such risks, dangers and hazards including the possibility of personal injury, death, property damage or loss resulting therefrom.

RELEASE OF LIABILITY AND WAIVER OF CLAIMS

In consideration of the Releasees permitting my participation in CanIL Activities, I hereby agree as follows:

To hold harmless and indemnify the releasees from any and all liability for any damage to property of or personal injury to any third party, resulting from my participation (including dependent children if applicable) in CanIL Activities;

1. This Agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns and representatives, in the event of my death or incapacity;

2. This Agreement and any rights, duties and obligations as between the parties to this Agreement shall be governed by and interpreted solely in accordance with the laws of the Province of British Columbia and no other jurisdiction; and

3. Any litigation involving the parties to this Agreement shall be brought solely within the Province of British Columbia and shall be within the exclusive jurisdiction of the Courts of the Province of British Columbia.

In entering into this Agreement I am not relying on any oral or written representations or statements made by the Releasees with respect to the safety of CanIL Activities other than what is set forth in this Agreement.

Page 3 - CanIL Photo Consent Form

CanIL and its partners produce videos, web, social media and print publications for the purposes of recruiting students, scholarship funds, and other special project support. We would appreciate your permission to use photos or video footage that may contain images of you.

This consent form only pertains to images of yourself (the signee). By signing this consent form below, you are NOT giving CanIL and its partners permission to use photos of dependent children. Photos of minors (anyone under 18) will never be used in any promotional material without explicit permission from their parents or guardians.

Please note that if you give CanIL permission to use your photos, the CanIL Communications department will still seek your permission before actually using specific photos or video that feature your image prominently. If you do not like the image, or would prefer that it not be used in a specific way, you will be able to decline at that time.

Please also note that due to the sensitive nature of where some students or staff work or are from, do not take or post photos of anybody without their permission.

Photo Consent

I hereby give general permissions for images of myself, captured during regular and special CanIL activities through video, photo, and digital camera, to be used for the purposes of CanIL promotional material and publications and waive any rights of compensation or ownership thereto.

Declined Photo Consent

Thank you for letting us know that you do not want images of yourself to be captured or used by CanIL. You will be put on a "No Photo" list, and the CanIL Communications department will not include your image or name in any CanIL public platforms.

Please also take personal responsibility to remove yourself from contexts where official CanIL group photos or videos are being taken, and let the photographers know that you would like to be excluded from any public use media. Beyond this, it is your responsibility (as needed) to monitor any casual photos that may occur in community with friends.

Notes and restrictions (e.g., "you can use my photo but not my name," etc.):

Restricted Photo Consent

Thank you for letting us know how you would like images of yourself to be used (or not used) by CanIL. The CanIL Communications department will abide by these restrictions.

Please also take personal responsibility to indicate any relevant restrictions to photographers in contexts where official CanIL group photos or videos are being taken, and monitor any casual photos that may occur in community with friends.
