Parent Match Request Form

Are you feeling alone?

There is a solution available in DC. The Family Ties of DC (FTDC) is an unique parent-to-parent support program designed to match a parent seeking support (Referred Parent) to an experienced parent (Support Parent) who can provide free and confidential support, information, and connection.  


WHO can connect?  DC Parent or Caregiver of any age of a child with a disability of any age.


WHY should you connect?  The advantages of a parent match include:

  • Gaining a sense of belonging, understanding, and trust with another parent

  • Identifying resources and supports to meet the needs of your family

  • Seeking respite and self-care supports

  • Learning strategies to advocate for your family

  • Becoming empowered to be a parent leader


HOW do you connect?  The connection is made through 1:1 parent match between a Support Parent who has child(ren) with disabilities with the Referred Parent who is seeking emotional support, information, and resources from a parent who has travelled their path.

Once the form is submitted, it will be sent directly to Rhonda White, FTDC Program Coordinator who will contact you no later that 48 hours upon receipt of the form.
