Thank you for your participation in CAN's food program!

To receive a food delivery, this form must be submitted and received by CAN staff by 1:00 pm on the day of food distribution. 

You will receive an email confirmation when this form is submitted, but you may call CAN's Green Baxter Court Community Center at (734) 622 9590 to check as well. 

We aim to deliver each order to the unit listed in the order form by 3:00 pm on the day of the distribution, however, due to unforeseen circumstances, deliveries could be late or unfulfilled. If this does happen, you are encouraged to call the community center so that we can address your concern as soon as possible. We will do our best to provide the items requested in each form, however, we cannot ensure you will receive what you request due to item and quantity limitations.

The food we receive for distribution changes frequently and so we are unable to give an exact list of what we will have available. We will do our best to provide what you need and alert you promptly if we do not have an item you requested.
