Please provide information pertaining to your private music study:





MYAC takes photos, videos and images for our website, flyers and brochures to publicize, market and recruit.  Please indicate whether you give us permission to photograph your student during the 2025-2026 season and waive your right to inspect and/or approve the finished product or the use to which it may be applied. 


All financial aid applications must be received by June 1, 2025. Here is a link to the form. Please send supporting materials to: jeanmilew@mya.org.  
IMPORTANT INFORMATION:
Once you have verified that all the above information is complete and accurate, please click the submit button below.  You will be directed to a secure on-line payment link for the $100.00 non-refundable application fee.  Alumni do not have to pay a registration fee. You will not receive a separate confirmation email to confirm registration. Your PayPal deposit receipt serves as your confirmation.
