At the Tottenham Hotspur Foundation we want our participants to have an enjoyable and safe experience with us. That's why it's very important for us to have clear and accurate information about people taking part. Please fill in the details below and take time to read the privacy notice and where we are asking for consent. 

For participants under 18, this form must be completed by a person with parental responsibility (legal authority) for them. 

PRIVACY NOTICE: This privacy notice tells you how we will use your information in line with the law and your rights. We collect personal information from you so that we can give you the services you have signed up for and to make sure that these services are run in a safe, fair and equal manner. We will keep your information secure and will not share data with anyone outside the Foundation unless requested by service providers and unless we are required to do so by law or to prevent abuse or harm. We will keep your personal information for the length of the course and a one year follow up period, after which we will anonymise so you can’t be identified any longer. 


Please note that those exhibiting at the Jobs Fair may seek info directly from attendees who visit their stands. It is completely up to you whether you share your personal information with those exhibiting at this event. We, Tottenham Hotspur Foundation, will keep your personal information for the length of the event and a one year follow up period, after which we will anonymise so you cannot be identified any longer. This can occur earlier at your request as you have a right as the data's subject.

CONSENT AND CODE OF CONDUCT
Image and case study consent

Images (photographs and/or video) may be taken on programmes and used to promote Tottenham Hotspur Foundation (The Foundation) or Tottenham Hotspur Football Club (The Club). Case study questionnaires or interviews may be conducted to celebrate achievements of the participants and demonstrate the positive impact of the Foundation and Club in the community.

These case studies and images may be used in internal reports and also appear in the public domain: in our printed publications (such as matchday programmes), on our website tottenhamhotspur.com, local or national media and used for other promotional material purposes by the Foundation and the Club.

All images will show participants in a positive manner. The Foundation will not permit case studies or images of participants to be used without their consent, or where appropriate, the consent of their parents or guardians.
